Conditions Accepted<\/label><\/li><\/ul><div id=\"wpforms-7-field_26-description\" class=\"wpforms-field-description\">By accepting the terms & conditions, you agrees\/consents to the following:\r\n\r\n1. To receive the service of broadband Internet provided by S.I.R. Tech Jamaica Ltd.\r\n\r\n2. To only use the broadband Internet service supplied for personal use AND will not\r\nreplicate and\/or offer any other personnel outside of this contract.\r\n\r\n3. To pay the standard, non-refundable, registration and installation fee for Internet service.\r\n\r\n4. That upon installation, the Technician will provide cabling, outdoor access point and\r\npower supply. If any other material(s) needed such as a stanchion, pipe, etc., it is my\r\nresponsibility as the customer to provide same to complete the installation.\r\n\r\n5. To pay on demand, the subscription fee which is due on the 28th of each month.\r\n\r\n6. To pay a security deposit of Six Thousand Dollars ($6,000.00) on equipment, that is, the\r\nrouter and antenna received upon installation. The deposit will be refunded providing that\r\nboth the antenna and the router are retrieved within fourteen (14) days of disconnection.\r\nNB: If there is an outstanding balance on the on the account upon retrieval within the\r\nstipulated timeframe, the deposit will be applied.\r\n\r\n7. That ALL equipment received upon installation remains the property of S.I.R Tech\r\nJamaica Ltd., hence, in the event of service\/contract termination; the equipment will be\r\nscheduled for collection in the shortest possible time by authorized personnel(s) of the\r\ncompany.\r\n\r\n8. To take care and protect the equipment received. This is solely my responsibility as the\r\ncustomer; hence, physical damage to equipment is the liability of the customer.\r\n\r\n9. That if there is an issue with the service, that is, the service is not performing as\r\nprescribed, I will contact the Service Provider immediately to diagnose and\/or repair the\r\nservice. Subsequently, with reasonable consideration, lack of service exceeding three (3)\r\nconsecutive days, after reporting service issue, an adjusted bill will be taken into\r\nconsideration by the Service Provider upon request.\r\n\r\n10. The Service Provider may experience problems due to natural disasters and\/or unforeseen\r\nevents, subsequently; S.I.R Tech Jamaica Ltd. will use its best endeavors to\r\nresolve\/diagnose the problem within an adequate timeframe.\r\n\r\n11. Physical damages to the equipment must be reported immediately, after diagnosis the\r\ncompany will advise customer of the cost(s) for damages incurred and repair\/replacement\r\ncosts.\r\n\r\n12. Late payment five (5) days after the due date will attract a fee of Two Hundred and Fifty\r\nDollars ($250.00). Non-payment for two (2) consecutive months automatically permits\r\nthe removal of all S.I.R Tech\u2019s equipment by an authorized personnel member;\r\narrangements will be made to retrieve the equipment.\r\n\r\n13. The Service Provider has the right to terminate the service upon giving thirty (30) days\u2019\r\nclear notice, if there is a breach of the contract, which includes but not limited to the\r\nservice being used for illegal activities. The customer is therefore expected to pay all\r\noutstanding charges\/fees.\r\n- 2 -\r\n\r\n14. Any special requests should be made to the S.I.R. Tech Jamaica Ltd. team where the\r\nproper protocol will be administered. Tech Jamaica Ltd has the right to terminate the service as a result of\r\nnonpayment of bill.\r\n\r\n16. As per condition seven (7), the provider has the right to authorize the collection of the\r\nequipment at any time deemed necessary with plausible reasoning.\r\n\r\n17. Once a customer\u2019s account becomes inactive for non-payment of bills, there will be a\r\nmonthly rental fee of fifty percent (50%) of the subscription until the bill is cleared or the\r\nequipment retrieved.\r\n\r\n18. Prior to installation and\/or a visitation the customer is entitled to a full refund, however,\r\nthere is a twenty-five percent (25%) service fee once there is a technician\u2019s visitation.\r\n\r\nSubsequently, if a refund is processed under any circumstances after a visitation, the\r\nservice fee will be deducted. Refunds are processed within four to five (4 \u2013 5) business\r\ndays. NB: Only cheques are disbursed for refunds and\/or reimbursements.\r\n\r\n19. Installations are usually scheduled and completed five to ten (5 \u2013 10) business days after\r\npayment is made and all documents are submitted. However, installations can be\r\nscheduled and completed before or after the standard wait time dependent on the\r\ncircumstances surrounding the installation.\r\n\r\n20. After completion of the installation, the Technician(s) will report the installation as\r\ncompleted; after, a request for activation will be submitted on the customer\u2019s behalf.\r\nActivations are usually done within twenty-four to forty-eight (24-48) hours of the said\r\nrequest.\r\n\r\n21. The Network Administrator continuously does upgrades to improve the service that we\r\nprovide; hence, unplugging the device will hinder such upgrades which may result in\r\nservice interruptions. It is recommended that the customer keeps the equipment plugged\r\nin at all times after the installation is completed unless instructed to do otherwise by an\r\nauthorized personnel member at S. I. R Tech Jamaica Limited. However, fear of damages\r\nby natural causes such as heavy rains, lightening, storms, etc., it is understandable if the\r\ndevice is unplugged.\r\n\r\n22. After disconnection, a customer is expected to clear ALL outstanding balances as at the\r\nlast bill\u2019s due date for the account to be reinstated. Reconnections are usually done within\r\ntwenty-four to forty-eight (24-48) hours of the account being cleared.\r\n\r\n23. After disconnection the Service Provider have the right to retrieve the equipment from the\r\npremises with or without someone being present.
